De gespecialiseerde trajectbegeleiding (GTB) 16+ wordt ingezet vanuit het team Persoonsgerichte Aanpak (PGA) van de directie Veiligheid, Bestuursdienst (opdrachtgever). De doelgroep betreft jongeren van 16 tot 27 jaar die in beeld komen bij politie en/of justitie waarbij er sprake is van multi-problematiek. Daarnaast vormt de jongere met zijn/haar gedrag een risico voor de openbare orde en veiligheid van de stad. De begeleiding kenmerkt zich door een hoge mate van inzet, betrokkenheid en flexibiliteit en is daarmee in de basis intensief. De omvang en intensiteit van de begeleiding verschilt per situatie en wordt afgestemd op de hulpvragen van de jongere, de complexiteit van de problematiek en het veiligheidsrisico. Het doel van deze begeleiding is in de eerste plaats het herstellen van het veiligheidsrisico en het doorbreken van het delict- en/of ernstig overlastgevende gedrag. Dit gebeurt door de jongere te begeleiden en te ondersteunen in het creëren van een stabiele leefsituatie.
