The goal is to promote the physical and psychological well-being of our employees through a three-tiered personal health assessment approach, that reflects the relative health risks and position of our employees: • Comprehensive health assessment for Executive staff and employees > 50 years • Mid-level health assessment for employees aged 40 – 49 years • Screening health assessment for employees ages < 40 years Ensure that the completed response form and proposal are included with your submission.
