Przedmiotem zamówienia jest świadczenie usług transportowych (dowóz i odwóz) pacjentów dializowanych z miejsca zamieszkania do Stacji Dializ Szpitala Powiatowego w Chrzanowie oraz z powrotem. Transport realizowany będzie samochodami Wykonawcy w systemie grupowym, z uwzględnieniem miejsca zamieszkania, stanu zdrowia oraz możliwości poruszania się pacjentów. Usługa realizowana będzie zgodnie z harmonogramem (Planem Dializ) zatwierdzonym przez Zamawiającego. Wykonawca zobowiązuje się do punktualnego transportu, zapewniającego niezakłóconą pracę Stacji Dializ.
