1. Przedmiotem zamówienia jest usługa druku materiałów edukacyjnych z zakresu profilaktyki wraz z dostawą w ramach projektu „Opieka koordynowana w Podstawowej Opiece Zdrowotnej”. 2. Przedmiot zamówienia został dofinansowany w ramach Projektu „Opieka koordynowana w POZ” w ramach programu Fundusze Europejskie dla Rozwoju Społecznego 2021-2027 współfinansowanego ze środków Europejskiego Funduszu Społecznego Plus (nr projektu FERS.04.15-IP.07-0003/23 3. Opis przedmiotu zamówienia zawiera załącznik nr 1 do SWZ. 4. Sposób realizacji zamówienia określa projekt Umowy stanowiący załącznik nr 2 do SWZ.
