Доставката на радиофармацевтичен препарат 18F-FDG е до товарния асансьор на Радиохимичната лаборатория на „ПЕТ/КТ сектора” в Клиниката по Нуклеарна медицина на УСБАЛО„Проф. Иван Черноземски“ ЕАД София. Активността на радиофармацевтичен препарат 18F-FDG, според заявената за деня по дати, да е както следва: ATC код Международно непатентно наименование (INN) Активност на опаковката Мярка Количество доставки за 24 месеца по мярка до: V09IX04 Fludeoxyglucose (18F) 5.0 GBq многодозова опаковка 30 V09IX04 Fludeoxyglucose (18F) 8.0 GBq многодозова опаковка 380 V09IX04 Fludeoxyglucose (18F) 10.0 GBq многодозова опаковка 50 V09IX04 Fludeoxyglucose (18F) 14.0 GBq многодозова опаковка 30 Прогнозно количество, общо за период от 36 (тридесет и шест) месеца – 735 (седемстотин тридесет и петдесет) многодозови опаковки. Срокът за изпълнение на поръчката е 24 (двадесет и четири) месеца, считано от датата на влизане на договора в сила. Възложителят предвижда опция за удължаване на срока на договора на основание чл.116, ал.1, т.1 от ЗОП, при същите условия, по негово искане, с не повече от 12 месеца след изтичане на договорения срок. В случай, че реши да се възползва от опцията, Възложителят писмено уведомява Изпълнителя за включването на опцията и срока, за който се удължава договора. Разглеждането и оценката на представените оферти, съгласно избрания критерий за възлагане се извършва от Комисията по реда на чл. 61 от ППЗОП, като техническите и ценовите предложения на участниците се оценяват преди разглеждане на документите за съответствие с критериите за подбор.
